On 8 May 2014, the chief officer on board the Danish tanker SCORPIUS was found dead on the bottom of a cargo tank. During a tank cleaning operation, the chief officer had inspected the ship’s cargo tanks, and in this process he took a few steps down each cargo tank access ladder. In one of the tanks, he fell from the top of the ladder and was killed by the impact of the fall.
The Danish Maritime Accident Investigation has prepared a marine accident report on the ocupational accident. The report centers on two topics: The construction and safety measures of the tank access ladders on SCORPIUS, and entering a tank without an entry permit.
