On 3 December 2013 there was a serious occupational accident on board the Danish tanker TORM REPUBLICAN in Bilbao, Spain. A crewmember was seriously injured when a mooring line parted just as he was passing beneath it.
DMAIB’s main focus areas in the investigation of the accident have been the conflicting goals that ship crews encounter and negotiate in their everyday work as well as the interrelations between regulations, procedures, crews’ perception of risk and the operational reality they meet.
